Update on Scan Forms for Evaluation

Scan forms continue to be a popular and efficient way to collect survey data for evaluation purposes. Over the past few months a number of modified and new scan form offerings have been uploaded to the Evaluation section of Organizational Development’s web page.

First, now there is just one cover sheet for all scan forms. The cover sheet should be completed and included with each set of scan forms sent to Organizational Development.

Second, extended (legal size) forms have been phased out. We’ll continue to process any that arrive for now but they are no longer offered on our web page. Legal size customer satisfaction forms have been replaced with letter size versions.

Third, ANR RPDs and Organizational Development have put together a set of new scan forms for the North and West regions (thus far) that lets users select common, region-specific indicators from pull-down menus – in addition to adding their own program-specific items.  This will greatly facilitate the measurement of regional and statewide ANR programmatic impacts yet still provide for customization of the evaluation to meet the user’s individual needs.

Finally, modified and new scan forms will be offered on a continual basis, so check the Organizational Development web page regularly. Links to the scan forms mentioned in this blog entry are provided below:
